Please help notes...

Many times services are initiated when a teacher or parent contacts me.  This year ('11-'12), I launched a new simple form for students to access school counseling services based on their own perceived need for assistance.  I targeted Grades 2-5 by introducing the form to each class and discussed with students in their some of the reasons kids might want to seek help.  Already, I receiving daily requests and some students who seemingly might not have sought help (when needed) have stepped forward with difficulties they're facing.  One student stated she liked the form telling me, "this was a really good idea".  I think so too. The "Please Help" form gives students a quick way to say they  need to chat. Students may place completed forms in a locked box.

I love the idea of incorporating a section on your objective board for, "I'll know I've got it when"

Classroom Decor Ideas: Students have to write in a log every time they do not turn in their homework on time and explain themselves. This helps teachers keep track of how many times the same student does not do his/her homework, makes students think about why they did not do their homework, and establishes accountability.

Objective board - I really like the "I know I've got it when..." aspect. It's more kid-friendly language and lets the kids know ahead of time what they need to be able to do to have mastered the content of the lesson!

This is a great way to display Learning Target, Performance of Understanding, Success criteria AND an essential question!!!

Exit ticket is a great formative assessment tool for teachers to leave as students are leaving the classroom. This allows teacher to see what students know or want to know more about. It can even tell the teacher what he or she needs to review.

I give exit tickets quite often. It can be time consuming especially with the more complex math problems when I collect the Math Ticket and have to check the students work prior to the next lesson. Having a technology based exit ticket would allow for the student responses to be easily organized.
