Writing Equations of Lines Foldable: There are 8 different scenarios: from 2 points, a point and a slope, from a graph, from its intercepts, from a slope and y-intercept, from a word problem, and from a line parallel or perpendicular to the line and passing through a point. Great review foldable covering writing equations of lines. Student quickly builds the foldable and can begin answering the questions in any order that they feel comfortable.

I love hands on activities like this when teaching slope intercept form.  This also makes a great test prep activity or end of year hands on activity when students are bored of regular math worksheets.
