Make-up Assignments Form - When students are absent, assign another student to record all homework assignments, collect the activity pages, and place everything in a folder with this form attached to the front. The folder is given to the absent student when he or she returns. When the work is complete, the folder is returned to the teacher with the work inside for grading.
