I could create this with each statement as a separate strip of paper, and then the kids can organize it as they decide who is responsible for what. I would also make some blank ones for us to add.  This would would be done in groups and shared as a whole class.

Students have to write in a log every time they do not turn in their homework on time and explain themselves.
