Trevi Fountain-if you miss this you really haven't been to Rome! It is always pretty crowded but well worth it. The sculptures are exquisite and the sound of the water soothing after a long day. We stopped by three times (it's in the middle of everything): morning, afternoon, and evening. It is beautifully lit at night--shadows exemplifying the mastery of the stonework. Be sure to walk around and get a good view from all angles. The horses are some of the best I've seen.
