They are just wooden squares from any craft store ($0.50 each) with metal house numbers I found at Shoppers Outlet for $2 each. These numbers were brass and spray painted Heirloom White. Put them up with finishing nails and use a sharpie to "paint" the nails black. Maybe could hang them with a wrought iron chain.

A tried and true method. Cut scrap paper to the size of the frame and mark the nail location on the paper. Using painter's tape, move the paper templates around the wall until satisfied. Then hammer the nail through the marks. Remove the paper template and hang!.... Genius!!!
